Tuition Assistance Form

Please answer the questions below, and return this form to the MSGL Office (2552 Soldiers Home Rd., West Lafayette, IN, 47906) by May 1st. If you have any questions please feel free to call: 765-464-1133.

We cannot consider your request without a statement of reasons for seeking assistance (line 6) and proper income verification. Please also fax the following for income verification to 765.464.8363:

Current paycheck stubs for each parent
Complete tax return for each parent for the tax year preceding your request, including all schedules and supporting documents

We also need an application and deposit to be considered for Tuition Assistance.
